CareBridge
Overview
CareBridge allows provides an easy to use method for ASC staff to both request and report structured information about care.
The system can be used to request information about individual service users, care providers or from members of the public. The responses can then be integrated directly into CareAnalytics records to improve analysis and planning.
CareBridge is accessed from the min CareAnalytics menu. There are two aspects:
- Send forms - Create a form and send for completion
- Review forms - View the status of forms that have been sent

- To analyse by service type choose Care bridge from the main menu.

In the Care Bridge menu choose Send forms
In Send forms you set up all of the information required to send out a survey request. Including which form is sent and who it is sent to. If forms are to be sent relating to specific service users then a cohort of service users must be selected that includes the service users that the from is to be sent to.

The information to be included is as follows:
Type of request. This can be:
- Single - a one off form request
- Standing - a request that sits in the recipients form responder and can be returned multiple times
- Not yet implemented Repeating - a request that is repeated at the specified interval
Send mode. This can be:
- One to each - where each chosen recipient will receive their own request
- One to all - where each recipient receives the same request and all can fulfil the request collaboratively (for example this could be used if there are multiple staff in an agency who might complete of contribute to a response). In this case each participant can partially complete the from and the content will be saved and available to view.
Assessment Tool
- Here you choose the survey to be requested
- This can be a pre defined survey, or you can create a new survey using the "+" button
Recipients
- Here you choose the recipients of the survey request. These can either be existing CareAnalytics users that you choose from the drop down list, or you can type in any email address.
Service users
- This option will only be highlighted if the survey you are sending relates to individual service users (e.g. a care delivery survey) in which case you select those service users for whom you wish the survey to be completed.
Once you have completed all of the information click "send".

All chosen recipient will then receive an email with a link to complete the requested form.
Process - Review Forms
- In the Care Bridge menu choose Review forms
- In Review forms you can see a list of all the requests you have sent and their status:
- The form sent
- The service user it related to
- The recipient of the form
- Who sent the form
- When it was sent
- last updated date
- Status
- Who contributed to the from completion
